For ASI, Distributor Central, PPAI, or SAGE Distributors requesting a sample, please fill out the form below and include the following information:

• The item(s) you are requesting, including any specific information (item color, imprinted or blank, etc).
• Ship-To Information (address, contact, etc)
• Your shipping account number (UPS or FedEx) and the method of shipping you are requesting.  If you do not provide a shipping account number or confirm the shipping method, the item(s) will be shipped via regular USPS Mail at no charge to you
• Requests can also be sent via email to our sample department at samples@yafaline.com.

Please note that due to the volume of requests, sample orders may take 2 to 3 business days to process and ship out unless otherwise requested and confirmed by Yafaline.
